OB/GYN Case Manager

The OB Case Manager will provide supportive services to pregnant and parenting women and their families.

Responsibilities: Responsibilities include intake assessment, service planning, service coordination and monitoring of patient progress from intake to discharge from services. Provide universal screening to determine social service and medical needs of the client. Develop and implement a process of reassessment for patients to determine ongoing needs. Provide appropriate education, counseling and services to women and families, refer clients as needed, coordinate care/service plan and follow-up client progress and work with a multi-disciplinary team to coordinate care. Conduct home visits as appropriate. Prepare internal and external reports for grant funder.

Qualifications: Bachelors Degree in social work, public health or other related field. Previous experience in case management preferred. Must have strong organizational and communication skills and be able to work with minimal supervision. Some evening hours. Bilingual in English and Spanish required.
