RN Care Manager

The Nurse Care Manager is responsible for medical review and clinical follow-up for new patients and coordinating care for identified patients.            

Responsibilities: The Nurse Care Manager interacts with patients to understand chronic and preventative health needs, communicates the care preferences of patients, serving as their advocate to help improve patient-centered systems; coordinate screening, services, and referrals with patients, with Patient Navigators. The Care Manager will also educate the patient/family/caregiver about the continuity of care management process and evaluate their understanding of the process, identify and address patient risk factors and/or obstacles to care and collaborate with Patient Navigator to ensure insurance coverage for coordination of care needs for patients

Qualifications: BSN in nursing with RN licensure; Minimum of three years of current clinical nursing experience with physician office experience preferred; Strong professional level of knowledge and comprehensive clinical assessment skills in the adult population and chronic disease management; EMR experience; Ability to work independently and perform multiple activities efficiently in a timely and accurate manner; Ability to work evening/weekend hours as needed; Bilingual English/Spanish preferred.
