<p>Anne Finucane is a visionary leader with the capacity to effect change at a global level and the humanity to touch individual lives through her compassionate philanthropy. Anne has been a hero to the South End Community Health Center on more than one occasion. Driven by her loyalty and friendship with Dr. Gerald Hass and Dr. David Urion, she has been a willing collaborator, a strong advocate and consistent source of financial support.  In good times and in bad, Anne has always taken the time to listen to our concerns and work with us to find solutions.  In 2008, Anne helped make history when, under her leadership, Bank of America collaborated with the Massachusetts League of Community Health Centers and Partners HealthCare to provide $5 million to support the Commonwealth’s loan repayment program for physicians and nurse practitioners who have made a two-to-three year commitment to practice in a Massachusetts community health center in exchange for medical school or nursing school loan repayment.   Over the years this program has enabled tens of thousands of hours of access to primary care for some of the state’s most vulnerable residents.</p>

<p>Family Medicine is a medical specialty concerned with comprehensive health care for the individual and the entire family. It is the specialty of breadth that integrates the biological, clinical and behavioral science. The scope of family practice is not limited by the patient’s age or sex, condition or disease.</p>
<p>Family doctors at SECHC take care of managing all your health care needs, whether you are well or ill. With an emphasis on long-term continuing care and disease prevention, we are here to serve you and your entire family; from infants, toddlers, school-age children, teens, young adults, pregnant women, adults and seniors. In short, we care for your while family, one person at a time.</p>
<p>Together with our Pediatrics Department, our Adult Medicine Department and our OBGYN Department, Family Medicine will offer a another choice to patients for their primary care at the center. 			<content:encoded><![CDATA[<p><strong>Children&#8217;s Health Insurance Program Reauthorization Act provides local support to those most in need of health care</strong></p>
<p><span id="more-314"></span>Boston, MA – (April 16, 2010) The South End Community Health Center has received a Children&#8217;s Health Insurance Program Reauthorization Act (CHIPA) grant in the amount of $304,385. The program, Los Niños Cuentan, allows the health center to make outreach, conduct enrollment and retain 1,500 uninsured Latino and homeless children within Boston into health care.</p>
<p><img src="http://www.sechc.org/en/wp-content/uploads/2010/06/big-latino-poster.jpg" alt="" title="big-latino-poster" width="300" height="450" class="alignleft size-full wp-image-597" />“This is an exciting program at a critical time, as the current State Children&#8217;s Health Insurance Program and Medicaid outreach and enrollment are not effectively reaching uninsured Latino children,” said Bob Johnson, Executive Director at the South End Community Health Center. “The project goals include outreach to the most vulnerable and underserved populations in Boston to facilitate access to health insurance and to primary healthcare services.”</p>

<p>There is a multipronged approach to reaching the constituency. Los Niños Cuentan will allow the health center to hire, train and deploy culturally competent community health workers (Promotoras) to engage community residents in community venues offering information, referral and follow-up. The Promotoras will target the Latino community for specialized outreach strategies; partner with grassroots community-based organizations to do on-site Virtual Gateway enrollments. In addition, a cadre of Latino volunteers familiar with the South End Health Center WIC, Pediatric and Obstetrics programs will use incentives for finding cases and referrals.</p>
<p>With enlarged enrollment, the South End Community Health Center will enhance pediatric practice and increase access to health center services. The Pediatric services will identify means to enhance capacities and orient services toward whole families to better retain children in primary care. There will be assigned case managers to families who are identified as at-risk of losing insurance or underutilizing primary care.</p>
<p>Each patient will receive holistic care by connecting those using WIC and Obstetrical services to Los Niños Cuentan Promotoras and Case Managers and to Pediatric services.</p>
<p><strong>About South End Community Health Center:</strong><br />
Established in 1969, South End Community Health Center (SECHC) is the largest provider of care to Greater Boston’s Latino population. More than half of employees at SECHC are bilingual in Spanish and English. SECHC is a community-owned and operated non-profit heath center governed by an all consumer Board of Directors. Serving a diverse population of 15,000 patients, SECHC users face significant barriers to care and have limited access to affordable, quality services. Sixty-five percent of SECHC clients have income levels at or below 200% of poverty, with 38% below poverty and 27% between 101% and 200% of poverty. Seventy-two percent of clients are enrolled in MassHealth or Medicare, 13% receive free care, 12% have private insurance and 3 % are self-pay. Forty-four percent of SECHC patient population is non-English speaking. SECHC’s annual budget is $8.4 million.</p>

<p>Dr. Nethersole is widely recognized for her visionary efforts to create collaborative initiatives designed to raise awareness of the problem and bring resources to bear to produce positive change and results where they are most needed – among Boston’s low income, culturally-marginalized, at risk children. The Fitness in the City program, a partnership between Children&#8217;s Hospital Boston and 11 Boston community health centers addressing the challenge of childhood obesity, is a prime example of her work. </p>

<p><span id="more-318"></span>Pablo Hernández Itriago, MD, a Primary Care Physician with special interests in preventive medicine, will lead the South End Community Health Center’s Medical Team as its founding Doctor, Dr. Gerald Hass, steps down.</p>
<p><img class="alignnone size-full wp-image-540" title="Pablo " src="http://www.sechc.org/en/wp-content/uploads/2010/06/200911_sehc919.jpg" alt="" width="675" height="450" /></p>
<p>&#8220;I’m very happy to turn the work over to a wonderful young doctor who has lots of experience as I step down from 40 years leading the medical direction of this health center,” said Hass.</p>
<p>&#8220;I feel very excited for the privilege to succeed Dr. Hass as Medical Director at the SECHC. &#8230;community health centers have been placed at the vanguard of health care reform and SECHC is well placed to take on the challenge to continue to provide excellent primary care to the community of the South End in Boston,&#8221; said Hernandez. &#8220;Dr. Hass embodies the center. He is the founder. He has remained true to his commitment to serve the community that approached him 40 years ago and asked him for health care that would include excellence and continuity. Walking through the health center, you quickly realize he has touched everyone there. He either has been the employee’s pediatrician, when they were little, or the pediatrician for their kids.&#8221;</p>
<p>Prior to joining the Health Center, Dr. Hernández was an attending physician at UMass Memorial Hospital in Worcester and Assistant Professor in the Department of Family Medicine and Community Health at the University of Massachusetts.</p>
<p>Dr. Hernández graduated from J.M. Vargas School of Medicine at Central University of Venezuela in Caracas, Venezuela with honors and distinctions. He served as Medical Director and Rural Physician in Venezuela and performed an internship and residency in Family Medicine and General Surgery at University of Connecticut.</p>
<p>He also served as Chief Resident in Family Medicine for the Department of Family Medicine at UConn. Additionally, he received the Resident Teacher Award from the Society of Teachers of Family Medicine and the “35 Helping Hands” Award for Family Health Center of Worchester for Community Health.</p>
<p>Dr. Hernández is board certified in Family Medicine. He is also member of the Society of Teachers of Family Medicine.</p>
<p>The South End Community Health Center is widely recognized as a leader in providing primary care services to a diverse population of patients from Boston. Originally a pediatric clinic, SECHC has grown to become a full-service institution with 15,000 patients, an $12 million budget and a 100% community board. SECHC is widely recognized for its outstanding preventive care strategies, especially those aimed at children and families.  SECHC was the first community health center in Massachusetts to successfully immunize more than 95% of its pediatric patients.  SECHC is culturally and linguistically accessible &#8211; over 55% of its staff are bilingual/bicultural in Spanish/English. Johnson, executive director of Community Health Connections Family Health Centers in Fitchburg, has been named to replace Tristram Blake as Executive Director of the South End Community Health Center. <span id="more-75"></span>Blake has been the lead at the health center for over 38 years along side its founder Dr. Gerald Hass.  Johnson will be the third director to lead the organization in 40 years.<br />
Johnson, a long-term resident of Cambridge,  founded CHCFC in 2002 as a medical clinic. In six short years, he expanded its services to include dental, behavioral health and low-cost pharmacy care at four locations &#8211; Gardner, Fitchburg and Leominster &#8211; as well as ACTION, mobile and land-based health services.</p>
<p>According to SECHC Board Chair Kevin Lee Hepner, “Bob&#8217;s experience managing growth, his familiarity with federally funded health centers and his commitment to community owned and operated health care make him the perfect person to lead SECHC and take it to the next level of care and responsiveness.”</p>
<p>Originally formed as a pediatric clinic in 1969, SECHC today is a full service medical facility with a $9 million operating budget serving 15,000 patients in over 65,000 visits per year. Under Blake’s leadership, the health center built a state-of-the-art facility that spurred the renaissance of the Washington Gateway district of the South End. Today, the facility is also home to Pathways to Wellness, a holistic care provider with whom Blake established a strategic alliance in 2007.</p>
<p>Throughout the decades, social change, evolution of the South End community, health care financing, homelessness and economic shifts have been the backdrop against which SECHC has grown. Despite the turmoil and challenges, SECHC has maintained its commitment to its mission to remain independent and community-owned to best serve the needs of residents.</p>
<p>Although Blake will officially step down at the SECHC 40th Anniversary Celebration on June 9 2009, Johnson has already begun to shadow his predecessor and will assume a full time schedule in May.</p>
<p>“I am thrilled by the opportunity to lead this outstanding organization through these challenging times and to the next level of service to the South End and Lower Roxbury communities.  Although Tristram leaves big shoes to fill, he also leaves a legacy of excellence in management, leadership and commitment that have paved the way for the next 40 years.”</p>
