Adult Medicine

The Adult Medicine department treats patients from the age of 18 to the elderly. We offer comprehensive primary care for our patients with a focus on prevention. We have a team model using physicians, nurse practitioners, RNs, medical assistants, pharmacists, a nutritionist, and some allied specialists in Podiatry, Neurology, and Dermatology. We perform some office procedures such as joint and soft tissue injections, wart removal, removal of some smaller cysts and skin growths. Dental Care

The South End CHC Dental Department offers comprehensive dental care for the entire family. We have 6 state of the art dental operatories equipped with digital x-rays and panoramic machine. Our 4 full time providers take care of dental needs of patients including cleanings, fillings, root canal treatment, crowns, dentures, extractions and teeth whitening. With partners from the BU Dental Pedodontic Department, we take care of children’s teeth and provide regular follow up cleanings, sealants and fluoride treatment. Providers are available from 8am to 6pm Monday- Friday and 9am-1pm Saturdays starting in the Fall. We also provide emergency walk-in services on Mondays, Tuesdays and Thursdays from 10am-12 and 1pm-3pm. View Providers

Eye Care

The Optometry department at South End Community Health Center aims to provide first-rate, technologically advanced, comprehensive eye care to all of its patients. Our services include routine eye exams, contact lens fittings, vision therapy and training, treatment and management of ocular diseases, specialized pediatric eye care, and laser vision surgery co-management. In addition, our fashion optical offers a wide selection of brand name eyewear, and our exam rooms feature state-of-the-art diagnostic and imaging equipment. Family Medicine

The department of Family Medicine at the South End Community Health Center, is the new addition to Medical Services. This comes as a response to the growing need of our population to have broad access to primary care. We are devoted to provide comprehensive health care to the individual and the family. It is a specialty in breadth that integrates the biological, clinical and behavioral sciences. We provide care to all ages, both sexes, each organ system and every disease entity. Mental Health

The aim of Mental Health Services at the South End Community health Center is to provide quality care to adult and child consumers. Our services include individual counseling , group counseling , family counseling, couples therapy and psychiatric consultation. Providers are licensed clinicians in psychiatry, psychology and social work. Our clinicians have a variety of specialties including expertise in trauma informed therapy. Many of our clinicians are bilingual in Spanish. Our services to children are both clinic based and school based provided at some Boston Public Schools. We are licensed by the department of public health. We provide services to Department of Mental Health Clients in partnership with Baycove Human Services at the Solomon Carter Fuller Mental Health Center in Boston. Our clinic is a training site for graduate interns in Social Work, Psychology and Nursing. Domestic Violence services are provided at the clinic in collaboration with Casa Myrna Vasquez. Welcome to the South End Community Health Center Nutrition department. Our nutrition services provide reliable and evidence based nutrition information to guide our patients and families in achieving a balanced and healthy approach to dietary and eating issues. This is accomplished by giving our patients access to practical nutrition information in a variety of formats. This includes information in either individual or group visits. Pharmacy

The Clinical Pharmacy Department works collaboratively with the medical providers at the clinic to provide comprehensive chronic disease state management services. Our chronic disease state management services focus on diabetes, cardiovascular disease, and smoking cessation. The Clinical Pharmacy Department staff is made up of faculty members from Massachusetts College of Pharmacy & Health Sciences, pharmacy residents and pharmacy rotation students. Programs

Weight Initiative Now (WIN)

A six-week interval pediatric weight management program for families of overweight children between the ages of 8 to 16. Program includes nutritional education, physical activities and case management support.

Homeless Initiative

A comprehensive primary care service for homeless adults and families. Program includes coordinated health care and case management assistance.

Peri-natal

A comprehensive program that provides pregnant women with support services to complement their clinical care. Program includes case management assistance to ensure coordination of services.

Pathways to Wellness

Pathways provides complementary therapies for our clients as an adjunct to their Western medical care. Working in partnership with community health centers and organizations, Pathways promotes a continuum of care for our clients in body, mind and spirit.

WIC

WIC is a nutrition program that provides nutrition and health education, healthy food and other services free of charge to Massachusetts families that qualify. WIC stands for, Women, Infants and Children.
